Records Requests

• To request health care records, please stop by our office and sign a Release of Records form. 
• Records will be reviewed prior to being released to redact information in the record that pertains to other persons not identified as the patient/client
• Records will be redacted any information that may be harmful to the patient/client.

Licensing Board Complaints

To file a complaint with your provider's licensing board, contact

Texas Department of Health and Human Services

• You can make a complaint to the Texas Department of Health and Human Services (“TX HHS”) by
• mail or fax, but TX HHS prefers that complaints are submitted online via the Texas Unified Licensure
• Information Portal (TULIP) or by calling 1-800-458-9858.
• These complaints are not allowed to be anonymous.
