GETTING STARTED

We accept most insurances. Before we can begin services, we ask that clients complete our intake forms so that we can verify coverage… the last thing you want is for you to begin services and find out that your insurance will not cover it and be left with a bill.

 

Many insurance companies cover mental health services. Please check with your insurance carrier to determine your benefits. You will be required to pay insurance copays and deductibles at the time of service. Our office can provide you with more specific information.

 

PAYMENTS

We accept HSA/FSA plans, MasterCard, Visa, and Discover cards, as well as cash, check, PayPal and Venmo.  

 

If needed, we can offer payment plans. Our office can provide you with more specific information.

 

RETURNED CHECKS

Not this this would ever happen to you ….  we charge a $25 fee for each returned check.

 

FORMS

Please come by our office during work hours at your convenience to fill out the intake forms to begin services. For your convenience we could also offer you the option of faxing or emailing the forms to you. You may choose to bring the completed forms to the office during work hours. For your convenience we have a secure mailbox on the outside gate if you need to drop off the paperwork in the evening or on a weekend.

 

If you choose to email or fax the paperwork to us, please know that we are not responsible for the security and privacy of your information as we do not have control over Internet and website security.
